About Something new is happening in the world of mental health. In recent years, professionals from across the varied mental health disciplines—psychiatrists, clinical psychologists, social workers, counselors, marriage and family therapists, and others—have begun to ask questions about some of the basic assumptions that form the very foundation of our work. At the heart of these questions is a growing doubt about the official diagnostic systems for mental disorder.

The Diagnostic and Statistical Manual of Mental Disorders (DSM) and the International Classification of Diseases (ICD) are the two official diagnostic systems currently in use in the United States and abroad. Since the late 1970s, these parallel manuals have loosely followed the “neo-Kraepelinian” system, a categorical diagnostic paradigm containing criterion-based descriptions of mental disorders and their corresponding symptoms. The neo-Kraepelinian approach was expected to elevate psychiatric diagnosis to the standards of general medicine, and after its introduction, diagnosis became increasingly central to clinical research and practice. Today, researchers and practitioners across the globe use the DSM and ICD on a daily basis to understand and communicate about the mental distress reported by their patients, clients, and research participants. Diagnostic Manuals and New World Billing Codes: Can We Get Along?

The answer is no. Explaining how this is so in a blog is not realistic. It reminds me of when I was struggling with long-forgotten skills in advanced research design and analysis: As difficult as the statistics were, I had to take the word of others that any given formula was valid. It would have taken years of calculus and several chalk boards to “prove” just one equation.

Since it is clear that everything that we thought we knew is backwards, upside-down and inside-out, we are learning that Democide (death by government) is the leading cause of human demise. Things do not exist for the reasons that we think they do. Why should the American Psychological Association (APA), or the World Health Organization (WHO) have anything to do with wellness, as we understand it?

There has been a lot of buzz this week in regard to the forthcoming Diagnostic and Statistical Manual of Mental Disorders, fifth edition (DSM-V) and the rejection of it by the National Institutes for Mental Health (NIMH). It seemed too good to be true and this may be the case. For one analysis of this debacle: DSM, NIMH on mental illness: both miss relational, historical context of being human.

[Edit – 5/12/2012: The manuals go where the money flows. As nice as alternatives may seem, if the energy of our current exchange system is not addressed in some grand way, we are in trouble. OR: Until money is dealt with, as the Merovingian would say: The alternatives do not have a reason]

It only takes a moment of trying to get through the different systems to see that they are not in agreement. Were the creators of these ridiculous systems this stupid, or has it been by design? This may be a distraction from the fact that faster than you can say Agenda 21, we are moving toward a global standard.

Feb. 9, 2012—Beginning Oct. 1, 2014 all entities, including health care providers, covered by the Health Insurance Portability and Accountability Act (HIPAA) must convert to using the ICD-10-CM diagnosis code sets. The mandate represents a fundamental shift for many psychologists and other mental health professionals who are far more attuned to the American Psychiatric Association’s Diagnostic and Statistical Manual of Mental Disorders (DSM).

Most psychologists were trained using some version of DSM. For other health care providers, the World Health Organization’s International Classification of Diseases and Related Health Problems (ICD) – which contains a chapter on mental disorders – is the classification standard.

Over the years, efforts to harmonize these two classifications have resulted in systems with similar (often identical) codes and diagnostic names. In fact, even if psychologists record DSM diagnostic codes for billing purposes, payers recognize the codes as ICD-9-CM – the official version of ICD currently used in the United States. Since 2003, the ICD-9-CM diagnostic codes have been mandated for third-party billing and reporting by HIPAA for all electronic transactions for billing and reimbursement.

What to keep in mind and expect in the future

CMS announced that there will be no grace period for the implementation of the ICD-10-CM. As of midnight on Oct. 1, 2014, any claims filed for dates of service (for providers) or discharge dates (for hospitals) on or after this date must contain ICD-10-CM codes. Because this is a HIPAA mandate, penalties for failure to comply will be enforced. Civil and criminal penalties may include heavy fines and imprisonment. WHO will be completing the preparation of ICD-11 at about the same time that NCHS will be implementing ICD-10-CM. However, it is highly unlikely that there will be a similar delay in ICD-11 implementation in the United States.

Through a series of annual updates over the first few years following ICD-10-CM implementation, the U.S. is expected to bring ICD-10-CM in line with ICD-11, so that the latest version of the ICD can be adopted smoothly and gradually without requiring a sudden and major change in the classification or how diagnoses are reported.

Spinal Fusion And The Matrix

As I have noted in other blogs, for unknown reasons, my neck broke in November of 2011. My upper right arm and shoulder were paralyzed. In April of 2012, my cervical vertebrae, C4-C7, were fused. Much movement has returned, but damage remains. During the past year that I have spent trying not to feel like I was in the Twilight Zone, I have lived the paradox of looking into seemingly as many odd things that I can find for comfort. One of these “things” has been the meaning of The Matrix Trilogy. After all of this time, I just discovered today that Keanu Reeves also underwent a spinal fusion procedure in the late 1990s, and was still recovering in the early stages of filming the first movie.

Months ago I formatted a blog in regard to cervical spinal stenosis, but for a variety of reasons, I have opted out of posting in it. Part of that may be due to the fact that I have too many blogs going. It seems that each “about me” section could be unique. Anyhow, just to put the energy out to the universe for anybody who is living with a spinal cord injury (that and top-notch treatment):

Prior to the pre-production, Reeves had suffered two-level fusion of his cervical spine and his legs had been becoming paralyzed, so he had undergone a neck surgery. He was still recovering by the time of pre-production, but he insisted on training, so Yuen let him practice punches and lighter moves. Reeves trained hard and even requested training on days off. However, the surgery still made him unable to kick for two out of four months of training. As a result, Reeves did not kick much in the film.[19]

The San Francisco Pride Committee held a meeting on Tuesday night. It was their monthly meeting and the Board of Directors was well aware that there would be individuals from the community that would be attending to express their outrage at the Board’s decision to not allow Pfc. Bradley Manning to be honored as a Grand Marshal at this year’s Pride parade. However, the meeting was held in a small conference room and members of the public and press were kept out of the meeting.

Chief Operating Officer Earl Plante told KTVU that the meeting was held to “really listen to the community, share their concerns and try to move forward in a proactive way.” That is public relations gobbledygook. He also alleged that, since the decision, “Threats have been made to board members and volunteers. That was why access to the meeting was limited.”

No evidence to substantiate this allegation has been made public by Pride. Nobody attending the meeting was informed by the Pride Board of any details related to “threats” and how the Board was concerned for their safety. So, Plante’s statement is disingenuous and malevolent, as it functions as a ploy to destroy the credibility of supporters protesting the decision.

I spoke with four individuals, who were at the meeting. There are also accounts from the meeting online from Rainey Reitman, Bradley Manning Support Network Steering Committee member; Michael Petrelis, a gay peace and social justice activist; Tommi Avicolli Mecca, a writer, singer, songwriter and activist. What follows is a comprehensive and unabridged presentation of what happened at the meeting and how San Francisco Pride continues to make this controversy worse.

A Week Of Manning Up To San Francisco Pride

Only a few weeks ago I felt alone in my sense of dread for whatever I had known as gay rights. Lamenting our seemingly increasing association with corporate pedophiles who advance their nefarious causes in the name of our benefit, I wondered if we would even begin to see the problems, let alone work on solutions. I realized that The Castro was purchased sometime shortly after the days of Harvey Milk, but this past year of coming to terms with our civil rights being hijacked by child rapists and corporate thugs has been quite difficult to bear. After the recent actions of San Francisco Pride in regard to Bradley Manning and their corporate boot-licking, I realized that I am in good company. This blog is a roundup of some of the wonderful things that have been said far better than I could. And, why the fuck not?

News reports yesterday indicated that Bradley Manning, widely known to be gay, had been selected to be one of the Grand Marshals of the annual San Francisco gay pride parade, named by the LGBT Pride Celebration Committee. When the predictable backlash instantly ensued, the president of the Board of SF Pride, Lisa L Williams, quickly capitulated, issuing a cowardly, imperious statement that has to be read to be believed…

How Did It Happen So Quickly?:

So apparently, the very high-minded ethical standards of Lisa L Williams and the SF Pride Board apply only to young and powerless Army Privates who engage in an act of conscience against the US war machine, but instantly disappear for large corporations and banks that hand over cash. What we really see here is how the largest and most corrupt corporations own not just the government but also the culture. Even at the San Francisco Gay Pride Parade, once an iconic symbol of cultural dissent and disregard for stifling pieties, nothing can happen that might offend AT&T and the Bank of America. The minute something even a bit deviant takes place (as defined by standards imposed by America’s political and corporate class), even the SF Gay Pride Parade must scamper, capitulate, apologize, and take an oath of fealty to their orthodoxies (we adore the military, the state, and your laws). And, as usual, the largest corporate factions are completely exempt from the strictures and standards applied to the marginalized and powerless. Thus, while Bradley Manning is persona non grata at SF Pride, illegal eavesdropping telecoms, scheming banks, and hedge-fund purveryors of the nation’s worst right-wing agitprop are more than welcome.

I don’t mean to imply that Bradley Manning is Bayard Rustin redivivus, or in any sense his spiritual or political heir. In fact, we know remarkably little about Manning, and a cloud of speculation, much of it absurd, still surrounds his motives. Even that pronoun “his” is questionable. (Speculation persists, supported by chats Manning apparently had with an inquisitive hacker, that she identifies as a trans woman and that advocates and attorneys are suppressing this fact: perhaps to preserve Manning’s “respectability” for the trial. In an attempt to respect the uncertainty, I alternate pronouns.)  The fact that Manning’s been held incommunicado allows everyone to project whatever politics, priorities, or fantasies they like on the mute figure. For homophobes, Manning is a disgruntled and untrustworthy gay man, a living argument for ask, tell, and expel queers from the armed forces. For military interventionists like Dan Choi and Peter Tatchell, he’s an emblem of the kind of inclusive army they’d like, one where all your government secrets will be safe if the officers just welcome the homos with open, loaded arms.
